Get fit and have some racquetball fun with CBRA. 

To apply for, or renew your CBRA membership, click here or on the Application Form to the right (requires Adobe Reader). Print and fill out the form, attach your payment, and mail or deliver it to CBRA (the delivery instructions are on the form). You'll be bouncing off the walls in no time!

You can fill it out electronically, right on the form. Simply click on the field you want to add information into (or just to the right of the name of the field). And then type in the information.

You can click on the appropriate playing level and shirt size.

Finally, you can add the value (example 35) into the cost field for the various programs and it will change it to a dollar amount and do a total calculation at the bottom.

Then hit print to print out the form.

The only thing that will print is the form. The instructions will not print.

You can save the form by choosing a save-as option.

For those who do not want to do it electronically, they can print out the form and simply use a pen to complete it. (Old fashioned way)
